What you will actually pay, and why the number moves
There are two pricing models, and clinics rarely say which one they use.
- Per unit. Botulinum toxin is priced by the unit, typically $10 to $20 in the US. A lip flip uses 4 to 6 units. That puts the product alone at $40 to $120. Clinics using this model often add a separate injection or consultation fee.
- Flat fee. One price covering assessment and treatment, usually $150 to $350. This is why a clinic quoting “$199” and one quoting “$12 per unit” can end up at a similar total.
Three things move the number beyond that:
- Which product. Botox, Dysport, Xeomin, and Daxxify are priced differently and dosed differently. Dysport units are not equivalent to Botox units, so comparing per-unit prices across products is misleading.
- Injector experience. A nurse injector, a dermatologist, and a plastic surgeon do not charge the same.
- Location. Metro pricing runs above rural pricing, sometimes by double.
The practical question to ask a clinic is not “how much per unit.” It is: what is the total, does it include the consultation, how many units are planned, and what happens if I need a top-up in two weeks.
Why published prices disagree so much
If you have searched this already, you will have seen $50 to $100 on one clinic page and $150 to $500 on the next. Both can be accurate. One is quoting product cost at a low-volume clinic; the other is quoting an all-inclusive metro price. Neither page usually says which.
That is worth knowing before you assume a cheap quote means a bargain, or an expensive one means better work.
What a lip flip actually does
A lip flip uses a small dose of botulinum toxin to relax the orbicularis oris, the ring of muscle around your mouth, along the upper lip border. When that muscle relaxes, the upper lip rolls slightly outward and more of the red lip becomes visible.
Nothing is added. The lip is not bigger; more of it is showing. That distinction matters, because it sets a ceiling on the result.

- Lips that are already full, where there is little left to reveal
- A long philtrum, which limits how far the lip can lift before proportions look off
- Anyone wanting a visible increase in size. That is volume, and this adds none
- The lower lip, which is treated far less often and with different considerations

Folklore vs evidence
- Often implied: A lip flip is a safer, easier, no-risk alternative to filler because “it is just a little Botox.”
- What is actually the case: It is a smaller intervention, not a riskless one. The muscle being relaxed is used for speaking, whistling, drinking through a straw, and closing the mouth firmly. Too much product, or product placed too far from the border, can produce a weak-feeling upper lip, difficulty pronouncing certain sounds, or an asymmetric smile.
- Importantly, it cannot be reversed. Filler can be dissolved with hyaluronidase. A neuromodulator cannot be undone; you wait for it to wear off, which takes two to four months. That makes conservative dosing on a first treatment more important, not less.
- A note on the dose: A lip flip uses a fraction of what a forehead treatment uses. Four to six units against ten to thirty. The margin between “subtle” and “too much” is correspondingly small.
